If you are interested in registering as a patient, please apply directly in person to the reception desk. A member of our reception team will ask you for your address. It is now a requirement that we ask for confirmation of your address by means of a utility bill/passport. (If you are able to bring your medical card, this will help us to process your application.)
The receptionist will confirm whether your address is within our practice’s allocated area. We invite patients joining the practice to attend a new patient medical with the practice nurse. This gives us the opportunity to take your medical history, check that you vaccinations are up to date and record any regular medications that you take.
Our practice area covers post codes 2-10 Greengates Ravenscliffe, Undercliffe, Thornbury, Laisterdyke, Thorpe Edge, Idle, Eccleshill and Bolton. We do have an outer boundary (see map). This allows patients who are already registered here and who move house from within the inner practice boundary to the outer practice boundary to stay registered with the practice if they wish. The postcodes for the outer practice boundary include LS28 5AA, LS28 7QU, LS28 7HG, LS28 8HA, LS28 8JZ, LS28 8EL and LS19 6DQ. Please note - The new outer practice boundary is for our existing patients. We do not at present accept new patients who live outside the inner boundary but within the outer boundary.

A Patient Guide to GP Services
If you wish to pre-register drop in to the surgery and request for a form to fill out. When you have completed all of the details, hand it back to us and we will verify your details, check your registration form and book you a new patient health check. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
